'This 70-year-old patient is rehabbing from a fractured proximal humerus (upper arm). Her range of motion (ROM) and strength have improved, and new exercises need to be introduced to maximize her success. The tall kneeling T is an excellent exercise progression for periscapular strengthening. The pulling motion originates from the shoulder blades (scapulae), and the middle trapezii and rhomboids are the prime movers. There needs to be sufficient lumbopelvic (core) stability to safely perform this advanced exercise, allowing for weight shifting with the moving sled. Always perform the T movement with the spine in neutral. Emphasis is placed on proper breathing and controlled movement. ✅ Neutral spine ✅ Breathing diaphragmatically ✅ Controlled movement Effective and efficient rehab is criterion-based, not time-based.
